REGISTRATION

The practice welcomes new registrations from those living within our practice area. To register as a patient ask at reception for details.  Newly registered patients will be invited for a consultation within 6 months of registering.

To protect your confidentiality we do not take registrations on line but you can download copies of our  new patient registration questionnaire. We will need this and your medical card to register you. If you do not have a medical card we can send you the NHS form GSM1 to complete instead. 

Our catchment area  includes Whitchurch, Rhiwbina, Pantmawr, Thornhill, Tongwynlais, Morganstown, Birchgrove, Llandaff North, Gabalfa and those parts of Heath and Llanishen immediately adjacent to Caerphilly Road. If you have a friend or relative staying with you they can also be registered temporarily for the duration of their stay

If you wish to choose to see a particular partner for some or all medical conditions please let the receptionist know and she will note this in your medical records.  We will always try to ensure that this is possible, but there may be occasions when it is not possible.
